A recommendation is not a handoff
The guide separates “identified” from “contacted,” “scheduled,” and “confirmed” so the patient, family, and team can see whether housing, care, medication, recovery support, monitoring, logistics, family work, and safety will actually operate after discharge.
The Firm supplements the existing team without replacing it
You may use the tool inside your own workflow and remain the primary professional. The checklist does not send responses to The Firm, create a lead, certify a provider, or establish a treatment relationship. If a family later requests implementation help, roles and authorized communication are defined separately.
A practical review sequence
Start early enough to resolve barriers before the discharge date.
- Assign an owner to every required item
- Move high-risk gaps beyond “identified” first
- Confirm appointments, medication supply, housing, transport, and safety
- Define information sharing and who convenes a review
- Print a clean copy for the patient and authorized participants
- Revisit the plan when risk, location, or level of care changes
Privacy by design
The current tool stores status selections only in that browser on that device. Users are instructed not to enter names, diagnoses, medications, or other sensitive health information. Do not use the printed output as proof that a service was independently verified by The Firm.

May I link clients directly to the tool?
Yes. The tool is public and does not require an email address or send checklist selections to The Firm.
May I place The Firm’s logo on my materials?
Not without permission. You may link to the public guide and identify it accurately as a resource from The Firm Addiction Consulting.
Does the guide replace clinical discharge planning?
No. It is an implementation aid. Qualified treating professionals retain responsibility for clinical recommendations and discharge decisions.
