Fentanyl and Opioid Intervention

Do not wait for “rock bottom” when the next use could be fatal

Fentanyl has changed the risk calculation for families. Illicit fentanyl may appear in counterfeit pills or other drugs, and a person may not know what they are taking. Repeated overdose, escalating tolerance, withdrawal, deceptive pill use, and mixing opioids with alcohol or sedatives require decisive, medically informed action.

The Firm organizes professional opioid and fentanyl interventions for families who need a clear plan, responsible treatment options, and immediate follow-through. An intervention is not emergency care.

OVERDOSE IS A 911 EMERGENCY

Call 911 immediately if a person is unresponsive, breathing slowly or not breathing, has blue or gray lips, makes choking or gurgling sounds, or cannot be awakened. Administer naloxone if available and follow emergency-dispatch instructions. Do not leave the person alone and do not delay emergency care to contact The Firm.

WHEN TO CONSIDER AN INTERVENTION

• Any known or suspected overdose

• Counterfeit pill use or uncertain drug supply

• Opioid use with benzodiazepines, alcohol, xylazine, or other substances

• Repeated detoxification followed by rapid return to use

• Refusal of medication or treatment evaluation

• Leaving treatment early or losing tolerance after abstinence

• Family members repeatedly paying debts, housing instability, or rescuing crises

• Children, vulnerable adults, driving, weapons, or workplace safety concerns

OUR PROCESS

First, we assess immediate risk, substances used, overdose history, naloxone access, medical and psychiatric factors, prior treatment, legal or occupational issues, and the family’s current response. We identify who should participate and what must happen before the intervention.

Next, we evaluate treatment options that match the person’s clinical and practical needs. That may include medical withdrawal management, medications for opioid use disorder, residential treatment, dual-diagnosis care, outpatient support, or a staged continuum. We do not direct every family to one facility.

The family prepares a unified message and specific limits. The meeting is facilitated with dignity and clarity. When treatment is accepted, transportation and admission logistics should be ready so momentum is not lost.

AFTER PLACEMENT

Admission is a beginning. Case management, family coaching, monitoring, recovery coaching, or sober companionship may help coordinate transitions and reduce the familiar pattern of crisis, rescue, temporary calm, and recurrence.

WHAT IF THEY REFUSE?

The family cannot control another adult’s decision, but it can change its own participation in the cycle. We help relatives define support that remains available for recovery, support that ends, and the next response to predictable situations. Boundaries should be safe, lawful, realistic, and sustainable.
