The Universal Discharge Guide
Build Your Life-After-Treatment Plan
Turn discharge recommendations into confirmed housing, appointments, medication continuity, recovery support, monitoring, logistics, family agreements, and safety planning.
A free implementation tool—not a shortened sales preview
Use the complete checklist whether or not you ever hire The Firm. It is designed for families, patients, treatment programs, therapists, psychiatrists, case managers, interventionists, recovery coaches, attorneys, trustees, and family offices.
The Firm can supplement an existing treatment team without replacing it. If building the plan is manageable but implementing it feels overwhelming, we can help coordinate the pieces.
Move each critical item through four states
Do not confuse a recommendation with an operating service.
- Identified: a possible next step has been named.
- Contacted: fit, availability, cost, and requirements have been checked.
- Scheduled: the intake, appointment, or admission has a date and time.
- Confirmed: the receiving party and patient have everything required for the handoff.
Privacy and appropriate review
This version saves checklist status only in the browser on this device. It does not create a clinical record or send answers to The Firm. Do not enter names, diagnoses, medications, or other sensitive health information.
Before any future version stores, transmits, emails, or shares health information, it should receive an appropriate privacy and security review.
Using this with a client or professional team?
The professional entry point explains how to incorporate the tool without replacing the existing treatment relationship or turning the checklist into a referral requirement.
This is the framework The Firm uses with families and private clients when preparing for life after treatment. It is free to use whether or not you hire us.
Private by design: entries remain on this device. They are not submitted to The Firm. Avoid entering names, diagnoses, medication names, or other sensitive health information. You may save checklist progress locally, print it, or clear it.
Your unresolved domains
Complete the checklist to see which parts of the transition still need implementation.
Identified is not connected. Move critical items from identified to contacted, scheduled, and confirmed before the current level of care ends.
Common questions
Can a treatment professional use this with a client?
Yes. It is designed to support—not replace—the existing treatment relationship. The tool does not send client information to The Firm.
Does marking an item confirmed prove the service exists?
No. The person using the plan remains responsible for direct verification. A status selection is a planning aid, not certification by The Firm.
Can I return later?
Yes. Use “Save progress on this device.” The checklist state stays in that browser until it is cleared or browser storage is removed.
